As a finalist, Discern Health stands out for its commitment to preventing falls, improving health outcomes, and lowering healthcare costs. Out of more than 1,500 submissions, Discern has been named as one of nine finalists in the rising star: longevity category, narrowed down from 16 quarterfinalists.
Discern Health’s Fall Prediction Model accurately predicts falls up to a year in advance using customer’s existing claims data. Our risk scores, risk score drivers and intervention strategies enable timely interventions to prevent falls, enhancing health outcomes and reducing healthcare costs, thereby supporting healthy aging and extended healthspan.
